Many people assume that a simple tooth restoration (a.k.a. dental filling or “pasta” in the Philippines) is enough to keep toothaches away, when in reality, the problem may already require a root canal. On the flip side, some also fear they need a root canal and immediately spend thousands when a straightforward restoration would already do. To help clear up this common confusion, we consulted Dr. Darwin Ferrolino of Variahealth, who shared the usual signs to watch out for.
According to Dr. Ferrolino, tooth restoration (like fillings or crowns) is meant to repair damage caused by cavities, fractures, or worn‑down teeth — usually when the pulp (the innermost part of the tooth) is still healthy. A root canal, on the other hand, is needed when the pulp is already infected or dying. This procedure removes the damaged pulp, disinfects the tooth, and seals it to prevent further infection.

Here’s a checklist straight from a dental professional:
TOOTH RESTORATION IS ENOUGH IF…
- You have old restorations that need replacement.
- The tooth is fractured but not involving the pulp.
- There’s no spontaneous pain (meaning it doesn’t hurt on its own).
- Sensitivity to hot, cold, or sweets is short‑lived and fades quickly.
- No periapical infection is seen on an x‑ray.
In short, if the pain is occasional and manageable, and the pulp is still intact, restoration is often the right choice.
SIGNS YOU MAY NEED A ROOT CANAL…
- Deep decay reaching the pulp.
- Spontaneous pain that keeps you awake at night.
- Pain that worsens when you lie down.
- Lingering sensitivity to hot or cold foods (lasting more than 5–10 seconds).
- Results from a vitality test showing the tooth is no longer alive.
- Presence of a recurring “pimple” or sinus tract on the gums, which appears, bursts, and comes back.
These symptoms signal that the tooth’s pulp is dying or infected, and a root canal is needed to save the tooth.
Price also varies depending on what your tooth or teeth needs. Tooth restoration is way way more affordable than a root canal which often reaches 5 digits, plus x-ray fees. For an exact quote, you may reach out to Dr. Ferrolino via:

Toothaches aren’t always straightforward, and guessing between restoration and root canal can lead to more pain down the line or more wasted money. The best step is to consult a board-certified dentist who can assess your tooth through clinical exams and x‑rays. As Dr. Ferrolino emphasizes, being aware of the signs and reviewing your dental health regularly helps you make the right decision for your future smile.
